Clinician Responsibilities

Important Notice Regarding GFWC Services in San Antonio, TX

Please be advised that Garcia’s Family Wellness Clinic, LLC (GFWC) is no longer providing services in San Antonio, TX. While I, as the Director, was responsible for managing the day-to-day operations of the clinic, each Licensed Professional Counselor (LPC) or Licensed Psychologist (Clinician) operated independently and was solely responsible for their own patient care, evaluations, documentation, and communication regarding patient transfers.

GFWC remains responsible for maintaining completed and submitted patient records in compliance with legal and ethical guidelines. However, each independent Clinician was given multiple notices of the clinic’s closure, along with ample documentation time to complete their evaluations, patient notes, and reports. If you have not received communication regarding your records, please note that GFWC cannot create or provide documentation that was not completed or uploaded into our Electronic Health Records (EHR) system by your Clinician.

To obtain your records, it is important to know your Clinician’s name, as outlined in the Patient Intake Forms you completed at the beginning of services. If you need to contact your Clinician, you may attempt to locate them via Psychology Today, where many mental health professionals independently advertise their services and contact information.

If you are unable to locate your Clinician, you may also contact the appropriate state licensing board for assistance in retrieving their contact information:

If you were receiving services from one of our two Interns and are unable to retrieve necessary documentation, please email info@garciasfamilywellness.com with the details of your request.

We understand that this transition may present challenges, and we appreciate your patience and understanding as we ensure all necessary procedures align with professional and ethical responsibilities.
